How can I get evidence of my IRS Payment Plan?

If your taxes are not paid off entirely and you are on an IRS Payment Plan, you can submit Form 433-D as evidence of your IRS Payment Plan.  The form should include the agreed monthly payment amount and the total owed. Here’s a copy of Form 433-D for you to fill out.
